How Do I…

Change My Beneficiary?

TMRS forms require a signature and cannot be submitted electonically. Download the form you need. Once it is filled out, sign it. Mail or fax (not both) the hardcopy to TMRS. If you fill the form out by hand, please use black ink. Do not use highlighter on forms.

Because life changes (death, marriage, having a child, divorce) can occur during your career with the city, it is very important to ensure your beneficiary designation is up-to-date. Beneficiary data is shown on your annual statement and can be viewed through MyTMRS or may be requested in writing.

After enrollment and prior to retirement, members submit beneficiary designation forms based on their vested status. “Vesting” is the length-of-service requirement for a TMRS retirement benefit. Depending on the city plan, an employee is vested after obtaining either 5 or 10 years of service. If you are not sure if you are vested, contact TMRS or your City Correspondent.

  • If you are not vested, fill out the Change of Beneficiary for Non-Vested Members (TMRS-0007) form.

    Complete the top portion of the form:

    • Full name
    • SSN (must be legible and match the number on file)
    • Mailing address
    • Employing city
    • Daytime phone

    Fill in the information about your Primary Beneficiary
    (limit 3):

    • Full legal name
    • Relationship to you (spouse or non-spouse)
    • Social Security number
    • Date of birth
    • Sex (male or female)

    If you wish to name a custodian for a minor child and/or designate alternate beneficiaries (limit 3), fill out the appropriate areas of the form.

    • Signature Required – be sure to sign and date the form.
    • Send the form to TMRS either by mail or fax.
  • If you are vested, fill out the Change of Beneficiary for Vested Members (TMRS-007V) form.

    • Spousal Consent: If you are married and designate someone other than or in addition to your spouse as primary beneficiary, your spouse must give his/her consent by signing the spousal consent affidavit in the presence of a Notary Public.
    • Send the form to TMRS either by mail or fax. Please note that completion of the alternate beneficiary page is optional for this form; however, if completed, page 2 must accompany page 1 when submitted to our system.

Vesting is an important time to review your beneficiary because if you die without having a valid beneficiary designation on file at TMRS, death benefits will be paid in the following order after in-depth review:

  • To your spouse; or
  • To your children if there is no spouse; or
  • To the last designated beneficiary if there is no spouse and no children; or
  • To the estate
